Before Your Visit
Please complete before your first appointment:
- Fill out the health and dental history forms we email you after you schedule
- Email any recent dental X-rays to frontdesk@kovalchukdds.com, or ask your previous office to send them to us directly
You'll notice the forms are thorough, and there's a reason for that. Your oral health is closely connected to your overall health. Many medical conditions and medications affect the teeth and gums, and some oral conditions can point to broader health issues. We carefully review everything you share, so please take your time and answer as completely as you can. The fuller the picture, the better we can personalize your care. Recent X-rays also give us a helpful baseline for comparison.
Completing the forms ahead of time also helps your first visit go smoothly, so we can spend our time together focused on you.
What to Expect at Your First Visit
Your initial appointment is designed to be thorough and unhurried. Dr. Kovalchuk takes the time to understand your dental history, current concerns, and long-term goals before recommending any treatment.
Comprehensive Examination
A thorough evaluation of your teeth, gums, jaw, and overall oral health. Digital X-rays provide detailed diagnostic imaging with minimal radiation.
Discussion & Education
Dr. Kovalchuk will walk you through his findings, explain any concerns, and answer all of your questions. You'll see exactly what he sees using intraoral cameras.
Personalized Treatment Plan
Together, you'll develop a plan tailored to your needs, priorities, and timeline. No pressure, no upselling — just honest guidance from a clinician who takes the time to get it right.
Insurance & Payment
We are not contracted with any dental insurance plans, but PPO plan holders are welcome as out-of-network patients. We will submit claims on your behalf and work directly with your insurance company to maximize your reimbursement.
